LONG DISTANCE AUTHORIZATION CODE REQUEST FORM
Long distance authorization (auth) codes are provided in accordance with "Long Distance Telephone Calls" Policy 2110, UBP. Requesting departments, as well as the individuals who are assigned auth codes are advised to review and abide by this policy. Requests are usually filled within one (1) to three (3) business days.

Auth codes are delivered to the requesting dean/director/department head/manager via e-mail. Please treat the auth code as confidential information in order to assure only authorized calls are billed to the department. Please refer to "Long Distance Telephone Calls" Policy 2110, UBP.
