forms, fill it out, and take it with you when you meet with your Arts

General Advisement Worksheets:
Which of the forms you use depends on certain
factors. Typically, one would use
the “Blue Sheet”.
Change of Major SheetUse this form if you are already in A&S and need to change your major or minor. Return to the A&S front desk for processing after the department advisor signs it.
Suspension Forms:
If
you have been suspended you must petition to return to Arts and Sciences
whether you have finished your suspension or you are appealing to come
back early.
Suspension Appeal FormUse this
form if you were suspended and want to appeal it so that you may
immediately return the next semester.
Return from Suspension FormUse this
form if you were suspended and sat out the required time prior to
returning
Enrollment Related Forms:
These forms are related to processes that effect enrollment or
registration
Overload Request FormUse this
form if you want to take more than 18 hours in a semester (or more than 9
hours in a summer session).
Foreign Language Proficiency FormUse this
form if you are fluent in a language other than English and wish to use
this proficiency to satisfy the Core and Group 2nd Language
Requirement.
