Which of the forms you use depends on certain factors. Typically, one would use the “Blue Sheet”.

Use this form to transfer into A&S – to be used in conjunction with your ePROGRESS degree audit.

Use this form if you were admitted to UNM before Fall 1999.

Use this form if you already completed a Bachelor’s Degree prior either here or at another institution.
  

If you have been suspended you must petition to return to Arts and Sciences whether you have finished your suspension or you are appealing to come back early.

Use this form if you were suspended and want to appeal it so that you may immediately return the next semester.

Use this form if you were suspended and sat out the required time prior to returning.

Use this form if you want to take more than 18 hours in a semester (or more than 9 hours in a summer session).

Foreign Language Proficiency Form

Use this form if you are fluent in a language other than English and wish to use this proficiency to satisfy the Core and Group 2^nd Language Requirement.
