My classroom-based assessment instrument assignment
is typed, in 12 pt font, with 1” margins.
This project (or significant portions of this
project) has not been turned in for a grade in another course.
Both a criterion-referenced instrument AND
a description are included, on separate pages.
The assessment is either a checklist, a rating
scale, or a rubric.
I developed the assessment instrument myself
-- for example, I did not copy it out of a book or from the internet. Any
resources I used develop this instrument are clearly indicated.
My assessment description includes:
The purpose of the instrument (written like
“This instrument is designed to assess…..).
The age or grade, subject area, and disabilities
of the students for whom it would be applicable.
The type of assessment (i.e. criterion-referenced)
and scoring system (i.e. rubric, rating scale, checklist).
Complete directions for administration and
scoring/rating.
