Six forms you must file
These forms are available online here, or from the Graduate Advisor and the Graduate Director (see below), who will help you fill them out and sign them before official submission:
Appointment of Committee on Studies (pdf)
File this as soon as you have chosen a three-person Committee on Studies, normally in the second semester of coursework.
Application for Candidacy (pdf)
This lists all the courses, including Master’s credits and transfer hours, which make up your required 54 hours. File it before or during the semester you take the Comprehensive Examination.
Comprehensive Examination Form (pdf)
File this two weeks before your Comps, indicating your intent to take the exam and the committee in charge of grading it.
Dissertation Prospectus Defense Form (pdf)
This form records the outcome of your Dissertation Prospectus Defense. File it with the Director of Graduate Studies, along with your prospectus, for final approval.
Appointment of Dissertation Committee (pdf)
As with the Committee on Studies form above, this form allows the Office of Graduate Studies to check the graduate status of the faculty you have chosen to serve on your committee. OGS must approve this form before a dissertation defense can be scheduled.
Announcement of Dissertation Defense (pdf)
The Office of Graduate Study requests this form two weeks before you intend to defend your dissertation. At the defense, more forms will be distributed and filed by both you and the Graduate Director.
