 How to Order the Blue Forms (Faculty Information Forms)

The IDEA blue forms correspond to the ICES red forms. IDEA refers to these blue forms as "Faculty Information Forms". Just like the ICES red forms, you order the IDEA blue forms by calling:
What do I have to remind instructors about?
IMPORTANT!!! Remind instructors about two items:
- To select between 3-5 objectives when completing their blue faculty information forms. (Note: if instructors have questions about selecting objectives, have them read the article: "Some Thoughts on Selecting IDEA Objectives")
- To visit the UNM IDEA Order Forms web page and select between the long student survey form or the short student survey form. (Note: if instructors have questions about which form to use, have them read the article: "Short vs. Long Form")
Deadline for Returning Blue Faculty Information Forms
Upon receiving the IDEA blue forms, return them as soon as possible so that ITS can send out the envelopes containing the student surveys. Generally, follow the same schedule you had established for returning your ICES red forms.
Deadlines for Returning Surveys
The deadline for returning completed student surveys to ITS is the same as ICES—1 week after the session ends or earlier if possible. Surveys turned in after the deadline may not be processed until the following semester.
