Forms and Guides List

Transfer of Copyright

Your final manuscript must be accompanied by an AIP Transfer of Copyright Agreement, completed and signed by at least one author. Please note this form is very similar to one used for publishing a paper in any one of several AIP journals. The authors retain significant rights, as indicated on the form. The form also accommodates various special situations, e.g., U.S. government employment or “works for hire.” No paper will be published in the proceedings without a signed and completed copyright form.

Important: Fax the form below to STAIF's organizers at (505) 277-2814. DO NOT fax this form directly to AIP. You may also scan and email the form to isnps@unm.edu.

Off Prints Order Form

Each paid attendee of STAIF-2008 will receive a copy of the published proceedings on CD-ROM. However, you may request hard copy reprints of your paper, also known as "off prints," directly from AIP. There is an extra charge for off-prints, the cost determined by factors such as quantity ordered and whether or not you would like color printing.

To order off-prints, print and fill out the form below and submit it with your manuscript. You will be billed directly by AIP.

Each presenter at STAIF-2008 is requested to complete a short biography through our online bio submission form. This information will be used by the sessions chairs to introduce you when you present at conference.

2008 STAIF Registration Form & Fees

STAIF's organizers work diligently to keep registration fees to a reasonable cost, and providing one of the best values among annual meetings in the field. Last year's cost for a full registration was $595, with special rates available for one-day, and student* registrations.

Covered within the full and student registration fees are a copy of the annual book of proceedings on CD, final program book, abstract book, admission to all technical sessions, plenaries, exhibits, and coffee breaks, plus one ticket to the annual awards banquet. One-Day Registrations receive all of the above benefits with the exception of a banquet ticket, which must be purchased as an extra.

*To qualify for student status, the attendee must be a full-time student at an accredited university or college and provide a copy of their Spring 2008 class schedule.

Permission Request Form

In the event that you would like to use previously copyrighted material in your paper, you will need to secure permission first from the copyright owner. These days most publisher's have a permission request form they prefer to use, available on their own websites. If you know the publisher who owns the copyright on the material you would like to include, we advise you to check out their site first before using one of the forms below.

If the publisher does not have a specific form and requests that you provide one, this form is AIP approved.
