Forms

Program of Studies

You must file a "Program of Studies" form (available from the department or OGS) listing the complete program of studies as approved by the Graduate Advisor. This form may not be submitted before completion of 12 hours of coursework. It must be submitted no later than the end of the semester prior to that in which you expect to graduate, and it must be approved by OGS before you may take the master's comprehensive examination.
After the Program of Studies has been filed, you may change between Plans I and II only with the approval of the Graduate Advisor and OGS, and you must submit a new Program of Studies.

Notification of Intent to Graduate

You must prepare a "Notification of Intent to Graduate" form (available from the department or OGS) early in the semester in which you plan to graduate. The form is submitted to the Graduate Advisor for approval and then forwarded to OGS. The deadlines for OGS to receive this notification are October 1 for Fall graduation, March 1 for Spring graduation, and July 1 for Summer graduation.

Submission of this form, however, does not ensure that you will graduate in that semester. Graduation is dependent upon the completion of all degree requirements by November 15 for Fall, April 15 for Spring, or July 15 for Summer. If you do not complete all requirements as planned, a new Notification must be submitted for graduation in a subsequent semester.

Plan I students must be enrolled for the semester (including the summer session) in which they complete degree requirements (usually the submission of the approved thesis to OGS). Plan II students do not need to be enrolled during the semester in which they take the comprehensive examination if fewer than three consecutive semesters (including summer session) have elapsed since their last enrollment. Students who have not taken the comprehensive examination within this time period will lose their graduate status and must apply for readmission and enroll for at least one graduate credit to regain that status.
