Worksite Supervisor’s Evaluation Consideration List

(Intern: Download the Evaluation Consideration Form and provide a copy to your Worksite Supervisor.)

Below are the evaluation questions to be answered by the Worksite Supervisor:

  1. Were shared goals met?
  2. Was a product produced, and was it up to standard?
  3. Was Intern’s work of a sufficiently high quality?
  4. Were additional skills or different skills than had been anticipated needed to complete the work or reach the goals?
  5. What was the best skill or characteristic of this Intern that was brought into the job?
  6. What was the skill or characteristic that you would recommend more practice at or more training in to improve Intern’s performance?
