Website Change Form

For all website changes, corrections, typos, or broken links:

  1. Please download the Change Request Form (filename olit_changeRequestForm.pdf, file size 184kb).
  2. Fill out the form and be sure to include the URL (link) to the page where the change is required. Please be as specific as possible.
  3. Please include files, such as images, pdf's or Word Docs, if need be. If you have many files, please zip(compress) them as one file.
  4. Email the form as an attachment to Annalisa, the OLIT Website GPA at .
