Payment Information and Method of Payment
All payments must be in U.S. dollars. Accepted payment method are by Visa, Mastercard, check, cashiers check, or money order. Registration forms must be accompanied by acceptable payment.

A confirmation e-mail containing detailed information regarding your registration will be sent to you upon receipt of completed registration and payment. All conference fees must be received prior to the commencement of the 2007 RMA-PCAPPA Educational Forum (October 10, 2007).
Cancellation/Refund Policy
For a full refund, written cancellation must be received on or before September 10, 2007. Cancellations received in writing after September 10, 2007 will be refunded 50% of the total cost of registration. No refunds will be authorized for cancellations received after October 1, 2007. Refund checks will be prepared and issued after the conclusion of the conference.
Advertisement/Sponsor Logo Submission
Please submit both a color and a black-and-white version of company logo/design for full-page, half-page, and quarter-page advertisement to mmonroe@unm.edu, electronically in high-resolution TIFF format no later than July 1, 2007.
