
Following are the steps to take in Radioactive Material procurement:
| 1. | Check to see if your Permit Holder is allowed the material under
their permit. If not, the Permit Holder must request to have the material
added to their permit. The request should include the radionuclide, maximum
activity, location of use, and the proposed use of the material.
NOTE: Procedures which generate MIXED WASTE are NOT ALLOWED at UNM until waste management issues are addressed! |

| 2. | Receive an Authorization Code (or Requisition #) from our office (277-2753). You will need to provide us with information (e.g., Permit Holder, radionuclide, activity, vendor, PO #, etc.) before a Code will be issued. | |
| 3. | Instruct the vendor to put this Code on the shipping paper or outside of package before shipping to UNM. | |
| 4. | Ship to:

| 5. | Delivery to your location will be accomplished the same day we receive the package from your vendor unless we inform you otherwise. |
