XII. REPORTING AND NOTIFICATIONS
The Radiation Safety Officer or designee shall determine when incidents involving radioactive materials or excessive radiation exposure to personnel are to be reported to the State or Federal Agency.

Other reports, such as radiation histories, reports on dosimeter readings, and radioactive material inventories are provided by the Radiation Safety Division.

The Radiation Safety Division has the responsibility of maintaining radiation histories, posting exposure reports, and other general record information on each individual.  These active records are continually updated until the individual’s records are terminated.  Terminated records are kept by the Radiation Safety Division.  After one year, these records may be stored at an off-site location.

Records of radioactive material transfers are kept by the Radiation Safety Division.
