Request from Outside Entities for UNM's W-9

W-9   Request for Taxpayer Indentification Number and Certification

The University of New Mexico (UNM) is tax-exempt as an instrumentality of the State of New Mexico under IRC Section 115(1). The Internal Revenue Code (IRC) provides that the exempt purposes of state universities include all the same purposes and functions described in Section 501(c)(3).

As a state goverment educational entity, UNM is not required to file a 990, Return of Organization Exempt from Income Tax.

Email requests for UNM's W-9 form to Taxation tax@unm.edu . Please include the requestor's name and their email address so that a scanned, signed W-9 in adobe acrobat pdf format can be sent to them. Otherwise, please include either a fax number or physical address where they want the form sent.

If you receive a paper request, you can forward it to Unrestricted Accounting Main, Taxation at MSC01 1260 or FAX it to 505-277-0101.

W-9 forms or substitute W-9 forms for the University of New Mexico may only be signed by authorized staff of Unrestricted Accounting Main. Current valid signature's for 2011 are Ava Lovell, Elizabeth Metzger, Doug Nelson, or Joy Germack.

 

The purpose of the federal W-9 form is a certification of UNM's name - as it is shown on its tax return, federal tax identification number, and to claim an exemption from backup withholding while attesting it is a US entity.

UNM only allows one of two vendor addresses on its W-9, federal identification number, 85-6000642.
The two W-9 addresses are;
1 University of New Mexico, Main Campus & Branches, MSC01 1300 or
Health Science Center, MSC09 5222.  

These two central locations of Financial Services will receive, stamp, and log the checks.  They will be responsible for forwarding the checks to the correct department.  It is recommended that your department require an attachment or an invoice number on the check to communicate where the checks should be forwarded to.
