Annual Report Instructions: 2010-2011

Individual annual reports are collected from the various departments and divisions by the Secretary of the University and are incorporated into bound volumes to become The Annual Report for the University. As part of the University’s permanent record, The Annual Report is essentially the only compilation of the year-by-year’s history of the University and its individual components. It may also be used as a working document by the President and other administrative officers. Two copies are required: one copy remains in the Office of the Secretary and one copy is kept at University Libraries.

Deadline for academic departments submittal to deans: October 28, 2011 (Does not apply to non-academic units)
Deadline for all reports to Secretary of University: November 18, 2011 (Applies to deans and non-academic units)
 
Period to be covered: July 1, 2010 to June 30, 2011
Number of copies: Two double-sided copies
 
All academic units submit to: Dean
All non-academic units submit to: Appropriate Vice President
Deans will include a cover report along with the departmental reports for submission to the Office of the Secretary.  All non-academic units submit reports to the appropriate vice-president for transmittal to the Office of the Secretary. Deans and Vice Presidents will keep one copy of their unit's annual report for their files and submit one copy to:

Formatting

  • Must be on 8 ½ x 11 white paper
  • Use both sides of paper (single-sided copy will not be accepted)
  • Margins (top, bottom & sides) must be one inch on all pages including attachments
  • Heading to include:
    name of division;
    the period covered;
    name & title of person submitting report
  • Do not bind or fold report
  • Do not page number in upper right hand corners. (This space is needed for numbering the entire bound volume.)

Content

  • Please keep your report brief and well organized. Do not submit superfluous materials such as brochures, calendars, class schedules, or flyers.
  • The following items should be covered:
    • Significant developments during the academic year, 2010-2011
    • Significant plans and recommendations for the near future. (Budgetary details should not be included.)
    • Appointments to faculty/staff. (Please give specific effective dates.)
    • Separations of faculty/staff. (Please give specific effective dates.)
    • Publications of the division; publications of individual faculty/staff. (Copies of annual supplements to the biographical record should not be submitted.)
    • Outside professional activities of staff members.
    • Outside sponsored research. (Please give name of sponsor, amount involved, purpose of grant, duration, etc.)
