Curriculum changes (the creation, deletion or change of/to a new or existing course or degree or certificate) may be initiated by any faculty member who teaches in the department for which the curriculum change is being requested, the department chair or the Dean of Instruction. 

As a matter of collegiality and protocol, it is assumed that the instructor initiating a curriculum change has discussed the matter with his/her chair and, if applicable, coordinator prior to proceeding with the change.


The following forms are interactive, i.e., they may be filled out on your computer and then printed.
