Student Application Checklist
ePAN for Students - This form is to be used for hires, rehires, continuations, terminations, transfers (from work study to student employment and vice versa), job extension, supervisor changes and annual salary increases.
**You will need to login with your UNM NetID and password.
**All students will be continued at their current rate!
ePAN Instructions for Students
Demographic Form - This form is to be completed by all new employees and
by students wishing to make changes to their personal information such as, home address, emergency
contact information, name changes, etc.
Auto-Deposit Form - This form is to add, change, or cancel a payroll
automatic check deposit. It can only be used for deposits that are for 100% of net pay.
Employment Requirements - Requirements students must meet in order to be processed
through the Student Employment Source Data Station.
2008 Form W-4 - This form is to be completed by all new employees and
by students wishing to make changes to their withholding allowances.
This form can be completed online.
Please print out a completed copy and bring it to our office along with any other required hiring
paperwork.
I-9 Form - This form is to be completed by all new
employees and employees whose previous job assignment with UNM expired
more than 90 days ago. Please fill out Section 1 only.
Section 2 must be completed by personnel in our office. Please
bring proper identification as indicated on this form. Hiring
paperwork will not be processed for students who do not have proper identification. For information regarding changes to the I-9 form please visit

Online FTE Change Request - Students who have and/or will have an active assignment with Graduate studies will fill out this form.
Student Job Description Form - This form is required for all new hires, continuations, and pay increases.
Time Sheet - Students are required to use the new standard time sheet availabe in both MS Excel and PDF formats.
Two Job Memo - This form is to be completed by students who have 2 or more
concurrent job assignments through the Student Employment Office.
