OGS peridocially updates the Plan of Studies Candidacy form:

Please go directly to the OGS WEB site: Go to Forms. Go to Graduation and download the current form being used. It is within the ACADEMICS link and entitled "Application for Candidacy for the Doctoral or MFA Degree" for doctoral students and Program of Studies for the Masters Degree (for MS students):
